چکیده
A 60-year-old Jamaican man with a history of diabetes mellitus and receipt of a deceased-donor renal transplant 11 months earlier presented with 3 days of left flank pain. He described associated nausea and a mild cough but denied fever, shortness of breath, weight loss, or other symptoms. He had no complications since his transplantation, and his immunosuppressive regimen consisted of 5 mg of prednisone, 4 mg of tacrolimus twice daily, and 1000 mg of mycophenolate mofetil twice daily. He also took glimepiride for his diabetes. The patient was retired, had no pets, and had not recently traveled. He had a history of an untreated positive tuberculin skin test result. Physical examination was remarkable for a fever (temperature, 38.6 C) and bibasilar pulmonary crackles. Significant laboratory results included a white blood cell count of 13,800 cells/lL, with 83% segmented neutrophils and sputum negative for acid-fast bacilli. A chest radiograph revealed bilateral nodular consolidations with cavitation in the lower lobes (Figure 1). A microscopic image of a positive culture is shown in Figure 2. What is your diagnosis?

Histopathologic evaluation of Verrucous skin lesions in renal transplant recipients
Background: Warts are common in renal allograft recipients (RAR) with reported incidence of 24% to 100%. These patients also demonstrate an accelerated malignant transformation of warts. Although in immunocompetent patients, the histology of common warts is quite predictable, this is not so in RAR. Objective: The purpose of this study was to evaluate the histopathologic findings of warty lesion...
متن کاملUnusual manifestation of cutaneous Leishmaniasis in a renal transplant recipient
Cutaneous leishmaniasis may present as unusual manifestations in renal transplant patients receiving immunosuppressive therapy. This misleading presentation, may delay the diagnosis and treatment. Moreover special caution must be taken in renal transplant recipients because of possible interactions between antimony compounds and cyclosporine metabolites. We report a 45-year old man with 5 years...
متن کاملFirst case report of Neisseria lactamica causing cavitary lung disease in an adult organ transplant recipient.
We describe a case of an adult organ recipient patient with a pulmonary cavitary lesion due to Neisseria lactamica, a harmless commensal organism that rarely causes human infection. To our knowledge, this is the first report of pulmonary disease caused by this organism and the second case of N. lactamica infection in an adult patient.
متن کاملCase report of invasive, disseminated candidiasis with peripheral nodular cavitary lesions in the lung
We report a case of invasive candidiasis presenting as multiple lung nodules and cavitary lesions with minimal pleural effusion. Candida infections of the lung are rare but can occur after hematologic dissemination of the yeast from other body sites, such as the skin and the gastrointestinal and genitourinary tracts. Here, we describe the case of a 56-year-old female with a history of end-stage...
